What happens after you refer
- 1Submit a referral with the member's information and appropriate authorization or consent.
- 2We review eligibility and program requirements and reach out to the member.
- 3When appropriate, a qualified nutrition professional helps determine the appropriate meal plan.
- 4Meals are prepared to the approved nutritional requirements and delivered to the member's home.
- 5Our team coordinates ongoing service and stays reachable for questions.
TriMeals does not make eligibility, authorization, or coverage determinations. We collect and review information, coordinate the appropriate next steps, and keep the referring party informed.
